Take a moment to think about your organization. Can you easily identify which patients are at the highest risk? Do you have proven, practical strategies to bring them to a state of optimum health?
Risk stratification is a process to dynamically identify and predict a patient’s health status, allowing providers to prioritize and manage high-risk and rising-risk patients that need intervention to improve wellness and lower their risk level. CTG’s EHR and Population Health experts can guide your journey to build and implement a risk stratification methodology and strategy, and explain the crucial role it plays in your organization’s success in a value-based care environment.
During the webinar, our experts will discuss:
The fundamentals of risk stratification
Current methods and tools for stratifying populations
The critical role of the EHR in building a risk model
How risk stratification sets the stage for population management
Identifying care interventions at all risk levels
Determining your strategy for high-risk patients and care management
The role of care transitions
This session will help your team understand how to prioritize initiatives based on ease of implementation, maximum return, and patient impact. It will demonstrate how varied roles across the organization must work in concert to achieve improved population health outcomes and cost-effectiveness, and how risk stratification is a key component of any successful population health and value-base care initiative.
